This is a big problem in 3rd world countries - esp. Africa and South America.
In well regulated countries it is not such an issue.

Researchers remove cyanide from gold leaching process
Researchers from Curtin University have delivered a technology that will significantly enhance the leaching rates for gold ore without using cyanide.
