Blue sapphire engagement ring...where to start?

I'd most certainly ask David Klass for a quote. Send him the inspo and you budget for the setting. Ask how close your budget will get you. Do bear in mind that the sides having diamonds will eat a wedding band. So, ask DK to suggest a change that would avoid that. Then, post your CAD here for input.
 
Adding to my post. I'm in agreement with other. Someone that works at a jewelry store and when asked about a blue sapphire presents you with a catalog of gems and setting will not be capable of making this inspiration ring. TBH it is unfair to even ask if they are not a trained jeweler who has a catalog of custom work to show you. You risk your friendship in asking and their feeling about trying to "help" you. I can't tell you how many posts start with "my friend made X" and end with tears and anger. Also, be sure they have actually set sapphires. If they don't set stones in house, the answer maybe no. Don't let them experiment with your stone.

Do look into insurance for your stone. In the US, Jewelers Mutual is the only company I know of that covers unset stones and stones while setting and set jeweler. Even if your Homeowners or other product is less, it can be worth getting JM coverage during this production phase (1 year). You can drop them after and they go with a less expensive option after that first year.

I'm hopeful one of the stock settings will work, but I really think it is work getting a quote from a custom jeweler with a track record. In addition to David Klass, you can get quotes from Wink at HighPerformanceDiamonds and ID Jewellery in New York.
 
Sorry! This is on my mind because it so lovely. This is how I'd adjust the design.

So, for the side profile, I'd aim at something like this. You have a solid shank over the finger. The pear "feathers" in your inspiration are above that shank to allow a flushish wedding band. The will raise the height of the ring, so lower the sapphire into the setting more to make the overall height more reasonable. for daily wear. Sorry for the crappy drawing!

The idea is to have the cured feathers from the inspo above the shank like this setting. Then, the pears area above that (purple).

If there was room, I'd echo the feather swirl in the gallery.
