What is up with this report? Would you trust it?

Aside from the fact that it is a cruddy photocopy, the report isn''t the GIA format I''m familiar with. Help, anyone?

There are 2 types of report, the full GIA report as in the example you posted and the GIA Diamond Dossier which doesn't feature a clarity plot, could be you are more familiar with the DD. Stones which carry a Diamond Dossier are inscribed for indentification purposes.
