I obtained a vs quality sapphire and it shows typical signs that it's natural and unheated, rutile silk inclusions and uneven color zoning. It does have a very uneven distribution of color and I was wondering if I should get the stone heat treated due to the extreme distribution of coloring in it. What are your thoughts?

It could still be heated, needles and silk inclusions do not mean it's not heated, nor does zoning.

To be sure, send it to a reputable lab.
 
Both heated and unheated blue sapphires can have even distribution of colour or show colour zoning. Colour zoning doesn't show only in unheated sapphires. How do you plan on heating it? The temperature required is very high and I don't know enough to assure you of what will change.
