I can't be certain from just a photo but it looks like a 'nat thwe' (polished by the spirits) spinel, which is basically an uncut octahedral spinel crystal that has been lightly polished. If so, it'll be from the Mogok Stone Tract in Myanmar.

To me it looks like a scandinavian style silver piece with a synthetic ruby mounted upside down.

I doubt it, look at the shape. Uncut spinel crystals don’t look like that. I suspect it is a synthetic ruby

Too clean and well cut, as well as in a shape uncommon to rubies or spinel. I suspect it’s synthetic.
