DIY placecards

does anyone have a good template?? i''d like to do double layered folded cards with the names printed on the front and the table printed inside...

I could be wrong, but I think that when you buy a kit like this http://www.michaels.com/art/online/displayProductPage?productNum=wd0158 it tells you how to set it up on the computer so that it will print correctly.
