How to include plated dinner options on RSVPs?

Hi! I have to tell the resort in advance how many chicken & steaks dinners I need. Has anyone done this? How do you word RSVP''s?

I was going to just list:
___chicken ____ steak

but if they mark one of each, I won''t know who''s getting what, so that''s why I added the M________ on each line. Like this:

The favor of your reply is requested by
September 1, 2007

M____________________
____Accepts ____will attend
____Declines
___Steak ___Chicken M____________
___Steak ___Chicken M____________

We had this issue with our invites - this is how ours looked:

Please reply on or before August 11, 2007

___Accept with Pleasure
___Decline with Regret

Please indicate initials next to entree choice:
___Chicken ___Filet Mignon
___Salmon ___Vegetarian


Some people actually followed the directions, others did not. If they didn''t, we guessed on their meal. Since most people are married couples, we figured they could just switch their plates if they wanted the steak and we gave them the chicken! We''re not going to bother calling because that will take too much of our time. But I guess you could do that also if they don''t follow the directions.

It worked VERY well. Everyone got the meals that they wanted, and we were able to give our caterers a fairly accurate head count.
 
We did ours like Fancy
 
I don''t have any experience with RSVP''s but just curious if you''re having fish as well, since some people might be vegetarian or non-meat eaters?

It''s a DW and we''re only having 20 people. Luckily, everyone eats meat.

I like this. It''s very easy to tell how to fill out. The others are a bit confusing, IMO.
