Informing people of a name change

What is the proper way of informing people of your name change? Do you include this as part of your announcement?

Do you mean friends/family or companies? I am confused. I think most people assume the woman is taking the man''s name (b/c as of now that is the most common tradition). I didn''t really formally tell my friends and family. For companies it is more tricky. CC/bills/insurance just changed it over the phone. Banks required a copy of the MC.
